Health insurance is important with the cost of medical care it is virtually impossible for most families to be able to cope with their medical expenses without the help of insurance.
There are various medical plans available. The most popular ones are operated with a network of doctors and medical care givers who work for an insurance company. Health insurance approval often seems like a confusing track with twists of jargon and blind curves. But underwriting departments, the areas that decide on health insurance coverage, have rules of the road that carefully guide the process. For each client, the underwriters evaluate the risk of insuring individuals and then decide on appropriate coverage and costs.
Use this guide to help you navigate the process for your business.
The major arteries
In general, commercial insurance plans fall into three territories: individual, small group or large group. The state of Florida defines small groups as companies with 50 or fewer employees and large groups as businesses with 51 employees or more. Risk evaluation is also regulated by the state. Depending on the policies, insurance companies may review applicants medical backgrounds and decide whether to accept or deny applications.
Lone ranger: Individual insurance coverage.
Any individual can apply for individual insurance coverage. One key distinction between Individual medical insurance and group coverage is that insurance companies can deny an application for Individual coverage. High-risk individuals may not be able to get this type of insurance. Price for individual health care plans is primarily determined by age, gender, benefit level and geographic location. Sometimes applicants with health issues are accepted, but they may have to pay a higher rate. For example, someone with moderate asthma would pay more than someone who does not have asthma. Other times, health insurance providers will exclude a particular condition from coverage for a particular period of time.
Caravan journey: Small group insurance coverage.
In order to qualify for small group coverage, an employer must meet minimum requirements. These include whether an employer is a legitimate business and, most important, verification that the employees truly work at the company. Small group pricing is based on similar variables, including dependent coverage, as Individual medical coverage. The weighted average of individual employees risk profile is aggregated to arrive at a total premium. Florida regulations prevent insurance companies from denying coverage to individuals within small group plans based on medical conditions. In other words, an insurance carrier would not be permitted to deny coverage to one employee in a firm while accepting others. While an insurance company may not be able to deny coverage they are permitted to adjust premium rates based on medical conditions associated with the individuals in the group. Fortunately, the State provides limits to the range that rates can be adjusted. Rates can not be adjusted more than15 percent than the standard rate. Additionally, insurance carriers reward employer with a healthy population by potentially reducing rates a maximum of 15 percent less than the standard rate.
Healthy individuals could end up paying more in a small group plan than on an individual plan because the rates are determined by the composition of the whole
group.
Rush hour: Large group insurance coverage.
Large group coverage has many similarities to small group coverage. Employers and employees need to satisfy similar eligibility requirements. Insurance companies are also prohibited from denying coverage to specific people within a large group plan because of a medical condition. Pricing within a large group is based on similar variables as small group in addition to consideration given to prior claims experience.
Improving your medical mileage
Business owners need to decide on their goals from a financial, employee retention/recruitment and social benefit standpoint and allow those objectives to guide their conclusions about company health care. Insurance prices can vary dramatically for similar coverage, so employers should shop around for the best value. Florida has historically enjoyed a competitive health insurance environment. The competitive environment provides for many choices and competitive rates.
Another consideration for business owners is what percentage of health care costs they want to take on versus how much they want their employees to pay. The more employees pay in monthly premiums and in out-of-pocket expenses, the less business owners need to pay. However, medical benefits can be used as a retention incentive, so it could be to an employers advantage to take on a large portion of medical costs.

The American Cancer Society has weighed in on the great American health care debate with a new study that claims people without health insurance are twice as likely to pass away than insured cancer patients. In fact, Dr Brawly, the chief medical officer of the society, says that uninsured people have a 60 percent greater chance of losing their battle with cancer. This study appears to prove that access to health care is a significant factor in survival. However just what sort of health care is difficult for the uninsured to access?
The first problem is probably that the uninsured do not have access to screenings. We have been told for years that early detection is critical to survival for many types of cancer. So uninsured people probably get presented for treatment after the disease has progressed, and this makes a favorable outcome less likely. Secondly, uninsured people will probably not be able to take advantage of the best treatment centers, but may have to use facilities that are already overwhelmed.
And just looking at the totally uninsured does not tell the whole story. Many people, even with group health insurance, are underinsured. One example of this are health insurance plans that limit outpatient benefits. Very expensive treatments, like chemotherapy, are often done as outpatient procedures. Once a patient has used up a small benefit, they might as well be uninsured! Sometimes it is even worse than being uninsured, because the presence of a group health insurance plan, even though it has been maxed out, can interfere with qualifying for state high risk benefits in some states.
This link, between survival and access to health care, is very striking for cancers that can be prevented or treated if they are detected early. Breast cancer and colon cancer are examples of these diseases. Some uninsured people may even avoid tests because they do not want to label themselves as uninsurable in the future.
While the debate continues about health care reform, nobody is holding their breath for a quick answer. And while some people may criticize the uninsured because they fail to protect their health, nobody thinks that a death sentence is appropriate punishment either!

The Environmental Protection Agency (EPA) has taken a position on acid rain. Their primary goal is to make changes to current rules and laws to resolve the issue of acid rain. Three environmentalists made national news by climbing to the top of a smoke stack in order to draw attention to the negligence of manufacturers in making the necessary changes to reduce acid rain.
Acid rain pollution is a problem that affects everyone. It affects earth and water and all the animals and plants which live on them. If man neglects and minimizes the threat of acid rain, his children and the following generations will have a bigger problem to deal with. New laws are created to control and hopefully reverse the effects of acid rain. The long term benefits of such laws will show if they justify their cost.

The amount of acid rain can be greatly reduced by such simple actions like lowering ones electricity consummation switching to other energy sources. More frequent use of public transportation or carpooling will also reduce the pollutants in the air.
The effects of acid rain are felt worldwide. Acid rain is mainly created by the combination of sulfur and oxide mixing with moisture for the sky. These pollutants are created into the air by fossil fuels, vehicle emissions, and factory exhaust. Acid rain is considered one of the biggest environmental problems faced by the world today.
Acid rain is also acid deposits resulting from sulfurs and oxides mixing with drops of rain or snow, and then dropping to the ground or rivers. Winds are also carriers of acid in the air, in a drier form. The effects of acid rain are dangerous because they are seen on water supplies, the environment, wildlife, and plant life. Acid rain will only be controlled by a global agreement and re-enforcement of laws to protect the environment.

Its easier to forgive than to forget. Thats how the adage goes. But for some people, they rather forget because they cannot forgive.
We all reach a point where we are stuck. The minute we realize this, we want to get unstuck but it is hard to move on. Something is holding us up and stopping our development. It is clearly blocking our way and holding us back. This dark cloud over our heads must be eradicated. If not, it would be hard for us to make the most out of the future.
The feeling is always there. It is rooted in the past. We realize sometimes that this affects our future. Now that you realize this, you feel frustrated, angry and fearful. You also feel guilt. Theres the anxiety that the bad stuff happens again.
The only solution you can resort to is to make peace with the past. This is how you can move on with your life. If you are willing to look at the situation you are in and do something to about it, then you initially want to move on and be happy.
You have to do the following in order to reconcile with the past:
1. Acknowledge the pain.
2. Eradicate the guilt.
3. Stop being the victim.
4. Unload the indignation and the anger.
5. Recognize that you are a compassionate and strong person.
6. Put the past in perspective and leave it there.
7. If you see a pattern, stop doing it. This will eventually lead to happiness entering your life.
Forgiveness is the process or the action to cease the resentment or anger towards another person who has committed a mistake. It also means removing the demand for the person to be punished. Forgiveness can be granted without a form of compensation or expectation. For example, a person can be forgiven when hes dead.
It is ethical for people to ask for forgiveness to the person they have offended. If the apology is genuine, then he or she may be forgiven. But sometimes the offense is too grave that you cannot blame the person who was offended to just forget that it happened.
All of us can forgive. But sometimes, it takes a whole process for us to do this. It is an internal procedure that nobody else can do but you. You can only forgive people if you recognize the need to do so.

Spinal stenosis is a condition that involves both the bony structures and the nerves of the spine.
The bony structures of the spine are the vertebrae. They provide structure for our bodies and protect the spinal cord, which carries messages between the brain and body. There are about 33 interlocking vertebrae and they are articulated in a way that provides support for the chest and abdomen while allowing us the flexibility to twist and bend. The spine has two forward curvescervical and lumbarand two backward curvesthoracic and sacral, which help it absorb mechanical stress.
The spinal cord travels through the spinal canal from the head to the lower back. The posterior longitudinal ligament lines the canal between the spinal cord and the back of the vertebrae, providing additional support and protection for the spinal cord. All messages between the brain and body are carried by the spinal cord in a top-down fashion. If the spinal cord is severed, you lose all sensation and function below the injury.
A pair of spinal nerves leaves the spine between each pair of vertebrae. They split off of the spinal cord and innervate the structures parallel to that vertebra. The nerves pass out of the spinal canal through the neural canal and exit the spine through the space between vertebrae.
Whenever the spinal canal or one of the neural canals is narrowed or obstructed, you have spinal stenosis. The symptoms of spinal stenosis depend on which nerve or nerves are being affected.
Cervical spinal stenosis occurs in the neck and can involve either the cord or spinal nerves. If spinal nerves are involved, you will have pain, numbness, tingling and/or weakness of the neck, shoulders and/or arms. If the cord is involved, you may also have symptoms affecting your legs, usually uncoordinated movement and difficulty walking.
Thoracic stenosis is rare because there is less curvature in the thoracic spine, and therefore less stress on the joints between vertebrae. The spinal canal is normally narrower in the thoracic spine, however, so it takes less obstruction to cause symptoms.
Lumbar stenosis is common, and involves the lower back. Lumbar stenosis causes pain in the lower back and leg and may also cause weakness in one or both legs.
Spinal stenosis can be caused by narrowing of the spinal canal, narrowing or obstruction of the neural canal or narrowing or obstruction of the space between vertebrae. Stenosis can be congenital or it can be caused by injury, tumor, medical conditions or degenerative changes due to aging.
In the early stages, anti-inflammatory medications, physical therapy and other medical measures take care of the symptoms. Ultimately, the only way to correct spinal stenosis is with surgery to relieve pressure on the nerves.
